Print this form, deliver it to your physician and have him send us a copy via fax at (512) 244-6231. We will make a copy for our records, give you one along with a medical receipt with the treatment codes listed after your visits. Send these documents to your insurance provider to see if they are willing to pay for your treatment(s). You can also call your provider and find out ahead of time what their policy is regarding payment for massage therapy services. Good luck!

Our goal is to treat our clients in the manner which would result in the maximum benefit with the fewest number of treatments possible. Every client is unique but our goal is the same. We strive to meet YOUR goals as quickly as possible with the lowest cost in dollars and time as possible. Because of this philosophy your total 'out of pocket' expense, even if insurance doesn't pay, can be significantly less than other 'covered' treatments especially when considering co-pays and your time.
